GUIDES ON FILLING UP THE FORM:

• NAME - (use your name + your artist mark/name;                                      no need for surname)
• EMAIL - by putting your active e-mail
• SUBJECT - will be filled as "APPLICATION" 
• MESSAGE - situate your AGE / GENDER / LOCATION ​/ WEBSITE (like the facebook, blog) and add your 10 PHOTO CREATIONS by using or putting the photo URL/links or much better just put your Facebook Crafts Album URL/links.
